This has happened to me 3 times. BUT it was with the use of vertical dropouts and a titanium skewer!!!

I replaced the vertical dropouts with horizonal and the skewer with a non-ti heavy one and it never happened again. It happened those 3 (3 different races) times while I was climbing a steep section of a race course. I yanked the skewer clean out of position and the wheel basically fell out of the vertical dropouts.

Horizontal dropouts, like aero seatposts, won't be around much longer below the VERY pointy end of manufacturer's product lines. They make no sense for 99% of riders, including the vast majority of today's tri'athlete'.

Agreed. And you know what makes no sense for 100% of riders? That's right, semi-horizontal dropouts. My Giant tri bike has them, and they combine all the disadvantages of horizontal dropouts (harder to wrench/remove wheel) with all the disadvantages of vertical dropouts (no flexibility in wheel position). WTH, Giant? (It's otherwise a great bike, though.)

Had screw issues on the Cervelo. They moved, got bent.... The stock scews are short and wimpy. Put in longer, steel screws and reduced the problem. Not thrilled with the overall advantage/disadvantage ratio on the horizontal drops.
